Using your dental plan

You can sign up for Health and Dental Insurance during Carle's open enrollment period, which occurs each fall for a January 1 effective date.

You may also enroll, disenroll, or make changes to your Health and Dental plan within 31 days of a qualifying life-change event, which includes marriage, divorce, birth or adoption of a child, death of my spouse or child, employment or termination of employment of my spouse, or a status change from part-time to full-time or vice versa.

  • Use the Benefit Change Form to report such changes. Supporting documentation must also be provided to Carle Benefits within the 31 day time frame. Please see the Life Events page to determine what supporting documentation is required.
  • Dependent Eligibility Documentation will be required if adding a spouse or dependent child to the
    plan.

The Carle Dental Plan is administered by Health Alliance Medical Plans and your benefits can be managed through Hally. Refer to How do you download the Hally App?
